Homecare Assessment

When it comes to finding just the perfect homecare match, chances are you may have lots of concerns and questions. We, in turn, want to be sure we are sending our caregivers into homes that are safe and appropriate as well.

By chatting together, and exploring your circumstances, it gives us both a chance to get all questions answered. You will probably want to know how our company works. We would like to know what services your loved one expects, needs, and what level of assistance is comfortable.

There is no cost and no obligation for our initial visit, the complementary in-home evaluation assessment. It usually takes about one hour and here are some of the topics that are addressed:

Safety

What is the home environment like? Are their pets, fall or trip hazards, wheelchair ramps needed etc. If you need information about who in the community, such as non-profits or service organizations, might be able to help you with home safety improvements, we have information about community resources.

Client History

We can discuss current and past health and medical conditions including mobility, memory, and behavior. Also your loved one’s interest, hobbies, activities, friends, goals, and motivation. What did they used to enjoy? What would they like to continue doing?

Client and Family Lifestyles

Each family is different in how they approach home care. We can discuss what is important for you.

Daily Routines and Preferences

Arrive early or sleep late, we can review times and days of the week that fit in with minimal adjustment.

Service Needs

We will review the full scope of services that are options and answer questions such as: Is transportation needed? What type of home cleaning chores are important? If meals are prepared, are there specific diet restrictions and more…

Caregiver Credentials and Personality

The relationship between caregivers and clients is pivotal to success. Hard of hearing? A caregiver with a soft voice might be a problem. Memory problems? A caregiver with Alzheimer’s certifications may be important. Let’s explore what attributes you want in a caregiver.

After evaluating your family’s home care goals we have a working profile that will aid our caregiver in knowing exactly what you would like. Based on this information, together we can browse our caregiver summaries for you to make some choices.

We have the same goal as you: to find the best possible care for your loved one!
